- Review the selection criteria and choose a candidate who you think meet or exceeds the criteria, and is NOT a current Board of Director or Officer.
- Obtain the candidate’s permission to nominate him/her.
- Obtain the candidate’s curriculum vita (CV) or résumé-Step 3 does not apply to Local Chapter Awards
- Make sure the CV or résumé is current and includes:
~ Current and past professional responsibilities;
~ Education, including nursing degrees, professional development, continuing education;
~ Memberships in professional organizations and committees;
~ Certifications, accreditations, awards;
~ Publications and presentations.
- Complete the Nomination Form
- Fill out every part of the form; applications can be downloaded at www.aphon.org
- Applications are electronic form fillable PDF documents. Type required information in yellow form fields.
- Ask 2 colleagues to write letters of recommendation-Step 5 does not apply to Local Chapter Awards
- Select colleagues who know of the candidate’s expertise and outstanding qualities, and who can provide different perspectives on his/her qualifications for receiving this award.
- Examples of colleagues are: peers, students, patients, families, employers, other heath care professionals.
- Letters should include:
~ Candidate’s name
~ The letter writer’s relationship to, and experience with, the candidate
~ Indication of why they support the candidate and how the candidate demonstrates excellence in pediatric oncology nursing
~ Specific examples of how the candidate meets the award criteria
- Nominations deadline extended to Tuesday, June 30, 2009.
