    Sandra Randall from Portland posted this question in Dec 2013, but the only reply that I can see involves EPIC.  I’d like to revisit and not re-invent the wheel if another center has made an “ER card” that works:

    “Does anyone have  an “emergency room card” for their patients.  We are trying to continue to work on developing an ER card that patients can take with them to their ER with info such as diagnosis, allergies, any specific info re their preferences for antiemetics, cvc access, etc.  Does anyone have something they are using and willing to share?”

    At Phoenix Children’s Hospital, we give our patients a card (size of a credit card) that says hem/onc.  On the back it states general guidelines of who to call –hem/onc attending and then protocol for blood cultures, ceftriax, etc.

    It is generic so does no have diagnosis.
