    Hello,

    Our team was able to create this type of data warehouse for our sickle cell and hemostasis & thrombosis patients in a “patient registry”. To build this through your EPIC, enter a service request for your Healthy Planet team for creation of a patient registry. You can then explore use of published clinical programs with your EPIC team (ie tools that already exist that meet your needs). 

    Also it is imperative to develop a clinic note with discreet data elements to feed into your patient registry (ie flow sheets) and work with your team to create workflows around data entry.

    We are looking to standardize our sickle cell disease visit documentation for providers (we use EPIC). Would love to have a one stop place to see preventative care and sickle cell complications listed – a place that the entire multidisciplinary team could edit.  Eventually would also like to have a transition program guide.

    Does anyone have any documentation tools that are working for you?
