    Magnesium replacement is given to Stem Cell patients especially those with prolonged Qtc , we try to keep the level around 2 or higher when pt has prolonged QTc , it’s weight based dosing but the MD should figure out the algorithm like all other medications. It’s outside our scope to recommend how much the patient should get. Also keep in mind TPN content of magnesium , sometimes if patient is on TPN but does not have prolonged QTC and the level is not too low then it can be adjusted on the next TPN. Hope it helps.

    I am doing an EBP project regarding magnesium replacement for patients on Cyclosporine and Tacrolimus and there is not much in the literature. My question is:

    Do patients who received an allogeneic HSCT who are now on Tacrolimus or Cyclosporine get replenished for low magnesium levels? If so what is the threshold in which they are replenished?<o:p>
