We give ch14.18 on the inpatient Hem/Onc/SCT floor.  We have yet to have a severe enough reaction (knock on wood) to warrent a PICU transfer or admission.

Where are patients given the CH 14.18 and HU 14.18 antibodies in your institution for the first time?
Do you have a grading system for patients with a potential for reaction based on history symptoms?
