    We also have an annual consent for the chronically transfused patient.

    At Children’s Hospital Colorado, we have a serial transfusion consent for the hematology/oncology patient population which is valid for one year.  It is loaded into EPIC so it is readily viewable.

    How often do you obtain consent for blood product transfusions? Once per diagnosis, every admission etc. Does this vary inpatient to outpatient? Thank you!!
