All of our scheduled Chemo admits come through our Ambulatory Chemotherapy Transfusion Center where they will do a Double height and weight by 2 RN’s. If a patient does not come through our outpatient center ( sometimes they come through the OR and PACU) than there is a 2 RN double height and weight done on the inpatient unit upon admission for chemo. If the patient has been admitted in between Chemo and needs to begin their next round, they will do a 2 RN double height and weight check prior to the Chemotherapy orders being written.

Can you please share your procedure for how you obtain weights and heights for chemotherapy administration and any safety checks you have in place?
