Our patients come back from surgery accessed. We had only one time that a patient was not accessed. We had a different surgeon doing the procedure

This is for the inpatient: When your patients go for a new port to begin chemo within a day or two, do they come back accessed? Recently, our surgery team has been sending them up deaccessed and our nursing staff will then have to access on a new sore port site. Is this something anyone else is experiencing? This is a new practice from what we have done for years. If anyone has a policy that addresses this, would you please share?
