    We use Beacon
    The orders are written by a care manager
    Fellow and Attending or 2 attending release
    Pharmacist checks and makes
    2 RNs check the orders one usually is in a resource role as a “Chemo RN” orders against protocol etc
    Then 2 RNs double check at the point of delivery 7 rights

    We are evaluating our chemotherapy administration double check process from the time that orders are written to the point of administration.  What process does your institution follow?  Currently we follow the below outlined process:

    • Fellow and attending write, double check, and sign
    • Inpatient charge RN and 2nd RN or Fellow double checks and releases to pharmacy
    • 2 pharmacists double check and make chemo then deliver to the unit
    • 2 RNs double check at the point of delivery

    Looking to benchmark with other sites.
