1) provider writes order for chemotherapy (including supportive meds & fluids) 2) care coordinator (nurse) reviews chemotherapy 3) chemotherapy and fluids are second signed by another provider 4) primary nurse verifies drug and dose 5) second RN verifies drug and dose, as well as patient identification at the bedside

We are evaluating our chemotherapy administration double check process from the time that orders are written to the point of administration. What process does your institution follow? Currently we follow the below outlined process:
Fellow and attending write, double check, and sign
Inpatient charge RN and 2nd RN or Fellow double checks and releases to pharmacy
2 pharmacists double check and make chemo then deliver to the unit
