    We routinely use Cetirizine for pre-medication for blood/blood products and chemotherapy.  We dose at 0.25mg/kg to a max of 10mg per dose.  Our hospital has significantly lessened the use of IV Diphenhydramine due to the unpredictable side effect profile – although we can still access it on our formulary.  Since we implemented Cetirizine pre-meds, I can count on one hand how many times I’ve needed to order Diphenhydramine!

    I have successfully used cetirizine in patients who had previous reactions while receiving blood products, IVIG, and chemotherapy. These patients were able to tolerate infusion successfully with cetirizine as pre-med. Reasons for not using Benadryl were due to excessive aggressive behavior, tearful, extreme pruritis or generalized intolerance of the drug.
