    First spike (one/all lumens +/- peripheral): All lumens. Policy is we can do pooled cultures for F/N kiddos; however, for BMT we do separate vials for each lumen. No peripheral cultures.

    Ongoing fever during culture rule-out (one/all lumens +/- peripheral):
    We reculture every 24 hours with continued fevers. Unless there are positive results, then we separate each lumen in to separate bottles

    New fever after a period of defervescence (one/all lumens +/- peripheral): Considered as first spike again

    And any other contextual information you believe is important: We change caps with each set of cultures.  Also, if we have a positive culture, our docs have us give Vancomycin (or whatever antibiotic they order) and then reculture after the new antibiotic is given.

    New fever after a period of defervescence (one/all lumens +/- peripheral): if afebrile > 48 hours will start process all over

    We’re having some renewed interest in discussing culturing practices at our institution. Our PICU, NICU, CVICU/cardiac stepdown and PHO/SCT teams are all fairly different in their views.

    What are your practices for:

    First spike (one/all lumens +/- peripheral):

    Ongoing fever during culture rule-out (one/all lumens +/- peripheral):

    New fever after a period of defervescence (one/all lumens +/- peripheral):

    And any other contextual information you believe is important.

    We do peripheral and all lumens for first spike, all lumens (no peripheral) for ongoing fevers (some nuances but generality), and will draw peripherals again with all if new fever.
