    We do.  I couldn’t get the Power Point for education or the policy to upload to this site so I sent it to you via email.  Nursing, pharmacy, IS and Hematology worked on this together.  We needed to make our ordering templates match our pump libraries and policies.  We made all administrations weight based in units/kg for the loading doses and units/kg/hr for the continuous infusions.  The Hem Onc department is required to follow these children as we are a general pediatric floor.  Hope this is helpful.

    Our hospital is currently looking at revising our policy on continuous heparin drips. Do any of you have continuous heparin running on the general heme/onc floor? Are there any special considerations (rate of drip, titrate, transitioning to another anticoagulant) that you have? Do you have education or a reference sheet for the nurse? Thanks for your help.
