    Our chemotherapy policy requires two heights and weights before initial chemotherapy. After the initial ht/wt the policy is a height monthly and a weight weekly. Our documentation is all electronic, so not a problem finding this information.

    We get a height/weight upon arrival to OP and IP.  The height and weight obtained needs to be compared to a previous height/weight within 30 days if the patient is receiving chemo.

    Hi,

    I am wondering if you have a policy determining the amount of time it is acceptable for considering the “height and weight” prior to chemo is “current”.   Our policy states to obtain current height and weight. While inpatient, do any of your institutions have specifics whether height and weight has to be done the day of chemo, week of, every 3 days, etc?
