At our outpatient clinic chemotherapy consents are good for six months. New consents are obtained if a child goes off study, new medications are added, or if treatment for that patient changes (new protocol).

How often does your institution obtain consent for chemotherapy once the initial one is signed?
Example: does your institution obtain new consents at different cycles?
Your feedback would be greatly appreciated. Thank you.
