    Great question. Mary Lynn, that’s an excellent structure to use. Thanks for sharing. We don’t currently have a standard approach like that and yours is straightforward and focused.

    The only other additional areas touched on in our reviews are personal hygiene considerations (I.e. The quality/nature of oral care and CHG treatments), whether or not we used teach back to address those if the caregiver had tasks delegated to them, and whether they had considerations about the cleanliness of the room (noticed staff doing high touch surfaces, routine room cleaning performed, etc.). 

    Our challenges are that caregivers are quick to advocate for our staff and they fear getting anyone in trouble. It’s a good problem given that our staff form excellent relationships with the patients and families, but it is a variable nevertheless. We do speak to continuous improvement and confidentiality. It’s a work in progress.

    I am collaborating with my Institutions Homecare team to conquer CLABSIs in our Hem/Onc population, and was wondering if you have specific forms that are filled out when a CLABSI does occur, primarily from the caregiver standpoint? We’re looking at various resources in attempts to develop something ourselves. Any and all information is helpful.
