    Hi Lauren,

    We have a CLABSI Root Cause Analysis form we complete after each identified CLABSI. This tool involves speaking with nursing, patients/caregivers, and any multidisplinary team member who’s been in contact with the patient within 72 hours of documented infection. Hope this helps 🙂

    Hi everyone,

    I hope you all are staying healthy and safe.

    I am collaborating with my Institutions Homecare team to conquer CLABSIs in our Hem/Onc population, and was wondering if you have specific forms that are filled out when a CLABSI does occur, primarily from the caregiver standpoint? We’re looking at various resources in attempts to develop something ourselves. Any and all information is helpful.
