Reply To: Continuity of Care from Inpatient to Outpatient setting
We also have our inpatient unit on the same floor as our outpatient unit. We have started to cross train our nurses to be able to work in both settings. We also have a Primary Care nursing model on our inpatient unit and are working on extending this model to the outpatient setting with the addition of inpatient NP’s to help facilitate the communication process.
