We document our calls in Epic EHR. I do have a form that I can share that I used when doing some data collection on our calls. It has time start/stop of call, ingoing or outgoing, what service the child was on, was it a clinical call needing a nurse or provider input or a miscellaneous or junk call (the triage nurse number tends to be the recipient of many non patient related calls). It also has an area to write info from the call ans well as check if you needed to fax or scan anything related to the call. I can email it if you are interested. 81/2 x 11. 4 call boxes per page, two sided.
