We use heparin on all CVADs, we use saline then flush with heparin 10u/ml (2mls for PICC and 3mls for Port). In the adult world we use the 10u/ml heparin for ports but only use saline for PICCs. I’m not sure if they have been finding an increase in occlusion rates as I only work in peditarics.

Honestly, the idea of not using hep on a CVL gives me heart palpitations.
We use hep and saline on all of our kids with mediports. 100u/ml (yellow hep) for any kid with a mediport over the age of 1 year. 10u/ml (blue hep) for any kid with a mediport younger than 1 year. For PICC’s we flush q25hr with saline and hep (100u/ml) if they have a standard PICC. Power PICC’s only require a saline flush q24hr and no hep. We don’t have any tunneled or broviacs right now. We never hep lock PIV’s, though when I worked in adult oncology it wasn’t unheard of to put 1ml of the 100u/ml hep in a PIV if it was going to be used the next day.

Bringing back a couple of questions that have been discussed before:
1. Has anyone switched to using Saline only flushes for central lines (any or all) and if so, would you be willing to share your policy/procedure
2. For central line flushing in the home-has anyone switched to using only Heparin flushes without saline, or have you gone to only Saline flushes.
