Description
Title: Implementing Toyota Production System Methodology to Reduce CLABSI Rates: A Quality Improvement Project
Presented By: Angie Blackwell, MSN, RN, ACCNS-P, CPON
Heather Christensen RN, BSN, CPHON
Overview of the Course: Presented at the 2025 Spring Education Exchange (SEE) this course presents how our institution used improvement science to tackle CLABSI rates in pediatric oncology. We used an 8-step problem-solving method to map out current processes, identify high-risk areas, brainstorm effective strategies, and discontinue ineffective ones. Patient bathing was pinpointed as a key issue, with half of our prior year’s CLABSI events linked to non-compliance with hygiene care. We implemented and evaluated tests of change, including designing a new admission education process, standardizing and simplifying the bathing process, and trialing a bathing tech role to assist with baths at convenient times for families. After implementing these tests of change, bathing compliance increased from 85% to 94%, and we exceeded our SMART goal of reducing the CLABSI rate by 45%, from 2.14 to ≤1.18 per 1,000 line days by June 30, 2024. We have seen sustainment of bathing compliance with our current rate at 94%
This Course will Explore:
- How improvement science and an 8‑step problem‑solving method were applied to reduce CLABSI rates in pediatric oncology
- Identification of high‑risk processes, including the role of bathing non‑compliance in CLABSI events
- Tests of change such as redesigned admission education, standardized bathing workflows, and the introduction of a bathing tech role
- Measurable outcomes, including increased bathing compliance and significant reductions in CLABSI rates
- Strategies for sustaining process improvements and maintaining high compliance over time
