Candidate Statement Position(Required)

Please provide a typed personal statement not to exceed 250 words. This statement should describe why you wish to run for a position on the APHON Board of Directors and should include a summary of your qualifications (e.g., leadership qualities) and goals for effective execution of this office. Your statement will be printed in the official APHON ballot and in other official APHON publications exactly as we receive it from you. References

Reference letters are required as part of the application process. The following two references should email their reference letters to the APHON Staff liaison, Hazel Iglesias by March 31, 2025 at hazel.iglesias@aphon.org.

Reference qualifications:

A member of APHON who has volunteered in APHON locally or nationally and can speak to how you can support APHON’s mission and strategic plan as a future APHON Board Member.
Any professional colleague who can directly speak to your leadership qualifications. Examples of colleagues could include a leader, educator, mentor, etc. from in or outside your organization.

Current members of the APHON Leadership Development Committee may not serve as a reference. It is your responsibility to instruct these individuals to submit their letter of reference.

I hereby consent to have my name placed under consideration by the Leadership Development Committee of the Association of Pediatric Hematology/Oncology Nurses for the open board position(s).

I have reviewed the position description, expected responsibilities, qualifications, and time commitments required for the performance of the position selected. If appropriate and necessary, I have discussed with my employer the position description, responsibilities, and time commitments required for the performance of the position I selected on this form.

I understand that once the APHON Leadership Development Committee has approved my candidate application that the two references are required to validate my professional qualifications for this position. I have provided the names of the two individuals who will serve as a reference for me on the application form. I will instruct these individuals to submit the Reference Forms online once the Leadership Development Committee has informed me that my candidate application has been approved. I will also provide these individuals with access to the Quick Fact Sheet of the position I am seeking.

I permit for the Leadership Development Committee to interview the individuals providing references to validate my professional qualifications for the position being sought.
