Instructor Requirements
- Conduct a minimum of 1 Provider course per year
- Maintain an average of 2.5 or greater on course evaluations
- Comply with APHON Pediatric Chemotherapy and Biotherapy Provider Course Policies and Procedures
- Maintain APHON membership
- Maintain a current APHON Pediatric Chemotherapy and Biotherapy Provider Card
- Maintain CPON® / CPHON® certification
