I hereby consent to have my name placed under consideration by the Leadership Development Committee of the Association of Pediatric Hematology/Oncology Nurses for the open board position(s).

I have reviewed the position description, expected responsibilities, qualifications, and time commitments required for the performance of the position selected. If appropriate and necessary, I have discussed with my employer the position description, responsibilities, and time commitments required for the performance of the position I selected on this form.

I understand that two references are required to validate my professional qualifications for this position. I have provided the names of the two individuals who will serve as a reference for me on the application form. I will instruct these individuals to submit the reference letters online. I will also provide these individuals with access to the Quick Fact Sheet of the position I am seeking.

At the discretion of the Leadership Development committee, I permit for the LDC to contact the references mentioned above to validate my professional qualifications for the position being sought. I agree that all information provided by these individuals to the Association of Pediatric Hematology/Oncology Nurses shall be kept strictly confidential and shall not be disclosed to me.

I permit publication of my photograph, candidate statement, the information provided on my candidate information form, and my Curriculum Vitae/Resume in official APHON publications, including but not limited to the ballot, JOPHON, APHON’s website and social media platforms.

I am aware of, believe I am qualified for, and if slated, am willing to accept the responsibilities of this position.
