Instructor Status Extension Requests

To maintain current Pediatric Chemotherapy/Biotherapy Instructor status, the following annual requirements must be met by December 31:

• Conduct a minimum of one (1) Provider course per year. 
• Maintain an average of 2.5 or greater on course evaluations. 
• Comply with APHON Pediatric Chemotherapy and Biotherapy Provider Course Policies and Procedures. 
• Maintain APHON membership. 
• Maintain a current APHON Pediatric Chemotherapy and Biotherapy Provider card.
• Maintain CPON^®/CPHON^® certification.
