About the New Resources
The FAPHON Application Guide is a new resource and provides you with details about each section and application requirements. You should use this guide as a worksheet to save your progress. However, your final application should be submitted via the FAPHON application form found below.
The FAPHON application contains 6 sections. Sections 1-4 should be completed through the FAPHON Points Worksheet and uploaded into the FAPHON application below.
The FAPHON Points Worksheet was designed and reformatted to facilitate auto calculation of the new point system for you as the applicant and for the Professional Development Committee to verify the points awarded.
Note: We want you to focus on adding your accomplishments and contributions into the points worksheet without worrying about formatting or formulas. Your application will be double-checked to ensure all applicable points are included before the application is presented for review by the Professional Development Committee. Sections 5 and 6 consist of your written statements about your leadership, unique contributions, and your future engagement. These written statements are to be submitted within the FAPHON application, along with your updated CV and an attestation acknowledgement.
